To register to vote you must fill out a Voter Registration Application Form. Voter Registration Applications are available from the Division of Elections, driver's license offices, state agencies that provide public assistance, libraries and many other public locations.

A form may also be completed online. THE FLORIDA VOTER REGISTRATION APPLICATION FORM REQUIRES AN ORIGINAL SIGNATURE. The completed online form must be printed out, signed, and mailed (Supervisor of Elections, Hon. Paul A. Stamoulis, P.O. Box 511229, Punta Gorda, FL 33951-1229) or brought to one of the three Division of Elections offices. Your application will be processed upon receipt, and a voter registration card will be mailed to you.

The online Florida Voter Registration Application Form can be used to begin the process to:

• Register to vote in the State of Florida.
• Change your name or address.
• Replace your defaced, lost, or stolen registration identification card.
• Register with a political party or change party affiliation.
• Update your signature

Deadline Information The registration books will be closed on the 29th day before each election and will remain closed until after that election. You must be registered for at least 29 days before you can vote in an election. If this is a "new registration", the date a signed voter registration application is postmarked or hand delivered to your county supervisor of elections office will be your registration date. If your signed application is complete and you are qualified as a voter, a registration identification card will be mailed to you.
